What is Case Management?

Improving participant's lives through individualised support, guidance and advocacy, access appropriate services and supports, and achieve their goals and aspirations.

NDIS Case Management is a service provided to NDIS participants to help them develop, implement and monitor their NDIS plan, and coordinate supports and services to achieve their goals and aspirations. It involves:

  • Assessment and planning of NDIS participants’ needs, goals, and aspirations, with a focus on individualisation and person-centeredness.
  • Coordination and management of supports and services to ensure that they are meeting the participant’s needs, and that their NDIS plan is being implemented effectively.
  • Ongoing monitoring, review and adjustment of the participant’s NDIS plan and supports, to ensure that they remain appropriate and effective over time.
